Condition

Sciatica

Sciatica is the shooting pain, numbness, or tingling that radiates from the lower back through the buttock and down the leg. It occurs when the sciatic nerve — the longest nerve in the body — is compressed or irritated, most often due to spinal misalignment or disc issues.

Common Symptoms

  • Sharp or burning pain from the lower back down one leg
  • Numbness or tingling in the leg, calf, or foot
  • Pain that worsens when sitting for extended periods
  • Difficulty standing up from a seated position
  • Weakness in the affected leg or foot
  • Pain that shoots down the leg when coughing or sneezing
  • Constant ache on one side of the buttock
  • Pins and needles sensation in the toes
  • Difficulty walking or a noticeable limp
  • Lower back stiffness that accompanies the leg symptoms

How We Help

Sciatica is one of the most painful conditions we see in our office, and it's also one of the most responsive to chiropractic care — when the source of the nerve compression is properly identified and addressed. The sciatic nerve is the largest and longest nerve in the human body, running from the lower lumbar spine through the buttock and all the way down each leg to the foot. When this nerve is compressed or irritated at any point along its path, it produces the characteristic radiating pain, numbness, or tingling that can make sitting, standing, walking, and sleeping incredibly difficult.

The most common causes of sciatic nerve compression are structural: a misaligned lumbar vertebra pressing on the nerve root where it exits the spine, a bulging or herniated disc creating pressure on the nerve, or a tight piriformis muscle in the buttock compressing the nerve as it passes underneath. Less commonly, spinal stenosis (narrowing of the spinal canal) or spondylolisthesis (forward slipping of a vertebra) may be involved. Each of these causes requires a slightly different approach, which is why accurate diagnosis matters more with sciatica than almost any other condition.

At Trinity Life Chiropractic, we start with a comprehensive examination that includes orthopedic and neurological tests specific to sciatica, posture analysis, and range of motion assessment. We use our INSiGHT scanning technology to identify where nerve interference is occurring along the spine. These scans give us objective data about the state of your nervous system — not just where you feel pain, but where the dysfunction actually originates. In many cases, the point of pain is not the point of cause. For example, you may feel the worst pain in your calf, but the actual compression is happening at L4-L5 in your lower back. Understanding this distinction is critical for effective care.

Once we've identified the source of compression, we develop a targeted care plan. For vertebral misalignment — the most common culprit — we use specific, gentle adjustments to restore proper position to the lumbar vertebrae and take pressure off the nerve root. These adjustments are not the general "rack and crack" that some people picture. They're precise corrections to the exact segments that are creating the problem. For piriformis-related sciatica, we address both the spinal component and the muscular tension that's compressing the nerve in the buttock. We also provide stretching and strengthening protocols to support the correction between visits.

Most patients experience significant relief within the first few weeks of consistent care. The typical pattern is an initial reduction in the intensity and reach of the radiating pain — meaning the pain retreats from the foot back toward the buttock and then resolves progressively. This is a good sign because it means the nerve is being decompressed. Some patients feel improvement after their very first adjustment. Chronic cases that have been building for months or years may take longer, but the trajectory of improvement is usually clear within the first several visits.

What we want patients to understand about sciatica is that it rarely resolves permanently without addressing the structural cause. Pain medication, muscle relaxers, and steroid injections may provide temporary relief, but if the vertebra is still misaligned or the disc is still pressing on the nerve, the pain returns — often worse than before. Surgery is sometimes necessary for severe disc herniations or stenosis, but it should be a last resort after conservative care has been tried. Many patients who were told they needed surgery have found lasting relief through chiropractic care. We also want to mention that sciatica during pregnancy is extremely common due to pelvic shifts and increased lumbar load — Dr. O'Brien is Webster Technique certified and uses gentle, pregnancy-safe adjustments to relieve sciatic pressure for expecting moms.

Common Questions

How quickly can chiropractic help with sciatica?

Many patients feel meaningful improvement within the first few visits — reduced pain intensity and less radiating pain down the leg. Full correction typically takes several weeks of consistent care, depending on the cause and how long the issue has been developing. The pattern of pain retreating from the foot back toward the spine is a reliable sign of progress.

Is chiropractic safe for sciatica during pregnancy?

Yes. Sciatica is very common during pregnancy due to pelvic shifts, increased lumbar load, and the effects of relaxin on ligament stability. Dr. O'Brien is Webster Technique certified and uses gentle, pregnancy-safe adjustments to relieve sciatic nerve pressure without any risk to the baby.

Do I need X-rays or an MRI before starting chiropractic for sciatica?

Not necessarily. We perform a thorough physical examination and INSiGHT scans at your first visit that give us significant diagnostic information. If our findings suggest a disc herniation, stenosis, or other condition that requires imaging for safe care, we'll refer you for X-rays or MRI before proceeding with adjustments.

What's the difference between sciatica and general back pain?

Sciatica specifically involves the sciatic nerve and produces radiating symptoms — pain, numbness, or tingling that travels from the lower back or buttock down the leg, sometimes all the way to the foot. General back pain stays localized in the back. The radiating pattern is the hallmark of nerve involvement and requires a different treatment approach.

Can sciatica come back after chiropractic treatment?

If the underlying structural issue is fully corrected and you maintain good spinal health habits, recurrence is much less likely. However, the same activities and postures that caused the original problem — prolonged sitting, heavy lifting with poor form, sedentary lifestyle — can create new misalignments over time. We recommend periodic maintenance visits to catch small issues before they become big ones.

Should I rest or stay active with sciatica?

Prolonged bed rest generally makes sciatica worse by allowing the muscles to weaken and stiffen. Gentle movement is usually better than total rest. We'll guide you on which activities are safe and which to avoid during your recovery, along with specific stretches that help take pressure off the sciatic nerve between visits.
